New student enrollment page at Purwodadi Adventist High School (Boarding)

The following are the steps of the New Student Registration (NSR) in SLAPUR:

1. Fill out the NSR Form: Visit the online registration link.  https://sysadmin.slapur.sch.id/psb.

2. Tuition Payment: Transfer to:

   - Adventist  School Purwodadi

   - A/C: 144-000-517-1605

   - PT Bank Mandiri, Tbk

3. Registration Steps:

   1. Click on the NEW STUDENT REGISTRATION menu.

   2. Fill in the HEALTH RECORD FORM. If in doubt, fill in with “-”.

   3. Click CODE PROCESS after filling in.

   4. Complete the New Student Admission Form with the requested information.

   5. Click SEND. Wait for notification if successful.

   6. Wait for notification after verification.

   7. Print and bring the Registration Form along with the following documents:

   - Proof of transfer

   - Elementary / junior high school diploma (ORIGINAL)

   - Elementary / junior high school report card (ORIGINAL)

   - Legalized copy of diploma

   - Copy of birth certificate

   - Copy of family card

   - 3x4 potrait (2 sheets)

   - Certificate (if needed)

8. Verification: Bring the form to be verified by Administration, Finance, Head of Boarding, Student Affairs and the Director of the Purwodadi Adventist School.

RETURNING STUDENTS

Steps for Retruning Students Registrations (RSR):

1. Fill out the OSA Form: Visit  https://sysadmin.slapur.sch.id/psb/halaman-ketentuan-prosedur.mu

2. Enter NISN: Fill in with your NISN.

3. Update Data: Update if necessary.

4. Click UPDATE: Save changes.

5. Print the OSA Form

6. Verification: Bring the form to be verified by Administration, Finance, Head of Boarding, Student Affairs and the Director of the Purwodadi Adventist School.
